Perhaps, just perhaps, President Trump made a conscious decision to vary his method of speech to make him more electable.
Other politicians have done it. If you listen to early speeches of Margaret Thatcher before she became prominent her voice is very different. She had voice coaching to make her delivery better.

I've always thought he sounded like he was just a stream of conciousness kind of person. Not that I'm an expert but nothing about his speech has ever made me think he had dementia/alzheimers/whatever.
Doubtful, because he talks (and tweets) just the same way now as he did when campaigning and transitioning. I really do not think he is capable of better any more.
The reason to suspect it is the change from how he spoke when younger to how he speaks now. He was never a genius, but there was a time when he was much more articulate, used bigger words and used them correctly, and could stay on topic within a coherent and grammatically correct sentence. Now he appears to need a prepared script on the teleprompter to do any of that.
