Transition: what would be a good first step?

I'd love to hear from people who have transitioned from male to female. I know that every situation is different, and that this a very personal choice and process. But I am still curious to find out what were your first steps and how quickly did the transition evolved. Thanks so much in advance for sharing your stories!
