It was curious that this was a thirty-minute presentation without questions. In the back of my mind, I wondered how Trump could not resist the temptation to show up, given he had all of them in one spot to address.

You're forgetting who called this meeting and who invited himself as an afterthought. The felon will certainly be the focus of attention. This isn't a routine meeting. It's the first of its kind, ever for all top echelon ranks to meet at one place.

Hopefully, it will be televised. I'd like to see why we put out a global call for an inperson meeting given our telecommuncations capabilities. Remember it was scheduled for thirty minutes without questions from the audience. So, it could have been an email... bluster evidently.

The main difference is in the scope and source of the authority:
A Lawful Order is an order that must be obeyed and meets specific legal criteria. It is issued by someone with the authority to give it (which can be a commissioned officer, a non-commissioned officer, or sometimes even a soldier with delegated authority). It also has a valid military purpose (related to mission, morale, discipline, etc.), but it does not require the commission of a crime or violate the Constitution, U.S. laws, or applicable international law. Only orders meeting these requirements are lawful ones.
A Direct Order is typically a verbal, immediate command given by a superior (often an officer) to a subordinate. While it carries the weight of authority, the critical point is that all direct orders must also be lawful.
In short: "Lawful" defines the content and legality of the order; "direct" describes the manner of its delivery (usually immediate and specific). A soldier is only required to obey a direct order if it is also a lawful order.
In other countries a soldier fulfilling a non lawful order colours a crime.
 
It's not my job to distinguish a lawful order from an unlawful order. Both will be given directly so that's irrelevant. The military have courses given over to working out what is unlawful.

In my country it's more simple. A soldier has to follow orders except they are against the law. Then following orders is a crime. Plain and simple.

I propose an annual physical fitness designed to confirm they maintain the exceptional level of operational readiness required. It essentially should have two parts, the Personal Fitness Assessment (PFA) and the Annual Fitness Test (AFT).
The PFA measures general physical attributes and is typically a timed circuit of press-ups and sit-ups, followed by a fast run, say 1.5 miles.
The AFT, or Combat Fitness Test (CFT), is a fully kitted, loaded march, where the individual carries a rucksack and weapon over a set distance, around eight miles, which must be completed within a strict time limit, say under two hours, with the weight carried depending on the specific role.
