I'd be more concerned about the Uric acid and other such substances upsetting the blood pH. You probably won't get a notable immune response to it.

As a specialist in the medical field, I would like to clarify some aspects of your answers:
1. Urine is sterile, yes, and unless someone has a urinary tract infection (UTI), it is safe. In WWI, they used urine as an antiseptic for wounded soldiers, they did not have ethanol...
2. The main component of urine (besides urine) is urea, and not uric acid. Urea does not stimulate the immune system (neither does uric acid) because it is a small molecule that is similar for all living things.
3. There are no antibodies in urine, as long as your kidneys are good and you don't have a disease in your immune system. The function of the kidney is to filter the waste from the blood, but to keep the useful things, including antibodies.
4. The real risk of piss enema is the trasnmission of bacteria from the anal canal to the penis of the giver, because the intestinal flora (bacterial population) includes some species, such as E. coli and other Enterobacteriaceae, that are susceptible of causing UTI (most of which progress through the ascending route, from the urethra to the bladder, and probably the kidney or the prostate). The risk is not 100%, and not all events of contamination will result in disease, and most of the disease events are mild, but it is useful to know. I would recommend, as far as I know, a water enema before penetration.

Don't mean to change the subject but I'm curious.
Shit it's nasty stuff and you hear it contains E. Coli (isn't that a cough drop) and Hepites and all this stuff.
Are you saying that the anal canal or intesteins can contain certian desease and bacteria that are harmful to others but that the person who has it in there system my not be sick from it themselves. Like they are carriers or something.
Would this be true for just bacterias or are desiese included.

Or are you just pointing out the obviouse, that if the person is sick take percautions.
 
Let me make it clearer with more details:
In the intestines there are a lot of bacteria, among which are E. coli species, which are there as a normal flora, meaning they do not harm your intestines. However, some of those E. coli may cause urinary tract infections if they are transferred from your intestines to your (or someone else's) urinary system.
The term carrier means that you have the bacteria at the right place, you can transmit it, but you do not show the disease.
In this case, you have the bacteria, at another place, but you can transmit it, and it may cause disease in another person.
Precautions should always be taken.
