What Differentiates Anal Rapists
From Vaginal Rapists?

Abstract: During the daily assessment routine of the Federal Documentation Centre for Sexual Offenders, important differences between rapists who commit their offence through anal penetration and other rapists became apparent. The hypothesis following these findings assumed that these anal offenders would show a more sexualized behaviour than other rapists.
The following differences between these two groups could be shown: violence and hypersexual desire. Anal rapists show a significantly higher degree in both areas, whereas, on the other side, no difference concerning psychopathic behaviour of the offenders could be proved. The preliminary conclusions that could be drawn according to these results express that anal rapists are more sexualized and, furthermore, they have a more obsessive sex life. In general, rapists do not constitute a homogenous group of criminals (Knight &Prentky, 1990; Quinsey, 1984). Their offences vary in many aspects such as location, time, degree of planning, degree of violence, and the type of penetration. Despite the existence of all these differences, rapists have been handled as a homogenous group for a long time.
This point of view changed latest with the development of the Massachusetts Treatment Centre Typology for Rapists (MTC:R) by Knight and Prentky (1990). This typology, however, still does not differentiate rapists by manner of sexual penetration (oral, anal, or vaginal). Although anal rape in women is defined as a sadistic act by some authors (Dietz,Hazelwood, & Warren, 1990), the sadistic type of the MTC-R3 typology, for example, does not take into account the type of penetration (Knight & Prentky,1990).
On the other hand, in the Massachusetts Treatment Centre Typology for Child Molesters (MTC:CM) (Knight & Prentky, 1990), the anal insertion of objects, or anal penetration of a female child, serves as an indicator for sexual sadism. Therefore, although the authors did not define anal penetration as a diagnostic criterion for the sadistic type in the rapist typology, they did define it as an essential criterion for the sadistic type in child molesters. However, if there is a link between a sadistic sexual perversion and anal penetration during the offence, no empirical data so far has been published confirming this hypothesis. Also, the DSM-IV (American Psychiatric Association, 2000) and the ICD-10 (Dilling, Mombour, & Schmidt, 1991) do not define sadism by any kind of anal penetration or anal insertion of objects. In the DSM-IV and the ICD-10, sexual sadistic fantasies and/or behaviours that inflict pain to others are required for the diagnosis, regardless where the injury happens or how the penetration is done.
In our daily routine assessment, we observed that heterosexual anal rapists seem to be different from vaginal rapists, in as much as they might be more sexualized. We asked ourselves, What could be the meaning of committing a sexual crime by anal penetration of a female victim?
One hypothesis was that anal penetration might represent a degradation of the victim, as it is described in the MTC typology (Knight & Prentky, 1990), where the anal penetration serves as an indicator for expressive aggression. Another hypothesis was that anal penetration during rape might represent a sadistic act as it is defined in Dietz et al. (1990). And last, anal rape might be observed more often in hypersexualized rapists.
Kafka (2003) suggested that a hypersexual desire disorder plays an important role in sex-offending behaviour by some rapists. Men who exhibit a high sexual outlet are more likely to try different kinds of sexual intercourse, and thus, also perform anal intercourse with consenting partners. Hypersexualized rapists, therefore, might also tend to rape by various kinds including anal.
According to Quinsey, Harris, Rice, and Cormier (1998), sexual offenders are violent offenders. Because anal penetration in female victims usually is a more violent act than vaginal penetration, we were especially interested in the question whether anal rapists might show higher scores in the psychopathy dimension (Hare, 1991), and whether we could find a difference in the occurrence of personality disorders.
Sexual offenders exhibiting a cluster B personality disorder according to DSM-IV criteria, for example, are known to commit more violent crimes (Eher et al., 1999). Stoller (1975) described suppressed aggression as a main factor contributing to the development of a sexual perversion.
Because our hypothesis was that anal penetration during rape might represent at least some kind of a paraphilia-related disorder, we were also interested in the self-perceived and objectively measured aggression in those rapists.
METHOD
SAMPLE
The sample was composed of 62 consecutively selected, imprisoned, adult, male, heterosexual rapists who had been transferred to the Federal Documentation Centre for Sexual Offenders (Vienna, Mittersteig) of the Austrian Ministry of Justice, between January 2002 and September 2002. Rapists who had transgressed male victims during their index offence were excluded from this study.
Anal Rape 483 Group 1 (anal rapists, n = 15) consisted of rapists who had at least one anal manipulation in their index offence (anal penetration or inserting foreign objects). Group 2 (vaginal rapists, n = 47)was defined by rape restricted to vaginal penetration or manipulation. Between these two groups, there was no difference in age and total amount of previous convictions and verbal IQ (see Table 1).
STATISTICS
Because of the unequal sample sizes and heterogeneous variances, the Mann Whitney U test was used. For this reason, not the raw values but the ranks have been compared (Clauss & Ebner, 1985).
INTERVIEWS AND PSYCHOLOGICAL TESTS
For diagnosing personality disorders according to the classification of the DSM-IV, the standardized interviewSKID IIwas conducted (Wittchen, Zaudig,& Fydrich, 1997). To gather information about the level of psychopathy of these rapists, a structured interview was conducted (Psychopathy Checklist–Revised—PCL-R) (Hare, 1991). Paraphilias were diagnosed following the classification of the DSM-IV (American Psychiatric Association, 2000).
The construct of hypersexual desirewas used as described by Kafka (1997). For measuring aggression, the German self-rating instrument Fragebogen zur Erfassung von Aggressivitätsfaktoren (FAF) of Hampel and Selg (1975) was administered. For measuring suppressed aggression, which is defined as aggression not obvious to the offender himself, the variable Aggression Potential of the Rorschach test (Exner, 1986), following the interpretation of Gacono and Meloy (1994), was 484 International Journal of Offender Therapy and Comparative Criminology

RESULTS
Hypothesis 1: Anal rapists show a higher degree of violence in their crimes.
It is obvious that anal penetration during rape is a more forceful act than vaginal penetration. Therefore, we also concentrated on finding additional signs of violence during the crime. In 67% of crimes committed by anal rapists, the victims were additionally beaten, whereas only 28% of the vaginal rapists beat their victims. This difference was significant (U = 238.5, z = –2.261, p = . 024).
Hypothesis 2: Anal rapists exhibit a higher prevalence of
cluster B personality disorders (DSM-IV). A slight, but not significant, effect (U = 258.0, z = –1.828, p = .068) could be shown concerning the distribution of cluster B personality disorders. Larger sample sizes might turn this effect to significance. As predicted, a cluster B personality disorder (antisocial, borderline, histrionic, or narcissistic) was
found more often in the anal rapist group (see Table 2).
Hypothesis 3: Anal rapists exhibit a higher score in the Psychopathy Checklist-Revised (Hare, 1991).
Comparing the two groups, we could not find any significant difference concerning the PCL-R total score (mean = 21 in the anal rapist group vs. mean = 19 in the vaginal rapist group). Moreover, there was no significant difference in the PCL-Factor 1 score (which measures interpersonal and affective symptoms of psychopathy) and also no significant difference in the PCL-Factor 2 score (which measures antisocial behaviour).
Hypothesis 4: Anal rapists are more sexualized than vaginal rapists, and they show more paraphilias (DSM-IV) or paraphilia-related disorders (Kafka & Hennen,1999).
Because rape through anal penetration is described as typical behaviour of sexual sadists by some authors (Dietz et al., 1990), we compared anal and vaginal rapists by the existence of a DSM-IV diagnosis of sexual sadism.We found 13% of anal rapists but only 4% of vaginal rapists were diagnosed as sexual sadists. This difference was not significant.

Hypersexual desire as defined by Kafka (1997) means a total sexual outlet of equal to or more than 7 orgasms per week. As a minimum the duration of such a period must exceed 6 months after the age of 15. Knight (1999) described the sexual drive to be significantly associated with various kinds of sexual offending. Arrigo and Purcell (2001) pointed out that compulsive masturbation works as reinforcement to the fantasy system, which could lead to sexual offences.
The comparison between the two groups exhibited a significant difference in the variable Hypersexual Desire (U = 171.0, z = –3.232, p = .001). Of these, 53% of the anal rapists but only 13% of the vaginal rapists showed a diagnosis of a hypersexual desire disorder defined by Kafka (1997).
Hypothesis 5: Anal rapists are less overtly aggressive and more muted aggressive.
In a self-rating instrument for aggression, anal offenders had a significantly lower score of aggression (U= 135.5, z = –2.618, p = .009). To make sure that this result was not biased by the self-rating, we also analyzed the variable openness, a lie scale of the FAF, showing that there was no significant difference between the two groups.
The aggression potential (Gacono & Meloy, 1994), on the other hand, in the Rorschach test (Exner, 1986),was significantly higher in the group of anal rapists (U= 99.0, z = –2.803, p = .005). According to these, we concluded that anal rapists show a higher degree of muted aggression but a lower degree of self-perceived aggression.
DISCUSSION
When conducting a preliminary comparison of two groups of heterosexual rapists (those who had engaged in anal manipulation of the victims and those with vaginal penetration or manipulation), we could find some significant differences.
Although there was evidence of more violence during the anal offence (as observed in a significantly higher engagement of the offenders in additional beating), there was no difference in the dimension of psychopathy between the 486 International Journal of Offender Therapy and Comparative Criminology

groups. Also, the prevalence of cluster B personality disorders did not differ significantly across groups.
In the group of anal rapists, we found a significantly lower score in the total amount of self-perceived aggression in the FAF, a self-rating instrument for aggressive behaviour. Because, on the other hand, anal rapists could be found to have a significantly higher aggression potential (Gacono & Meloy, 1994) in the Rorschach test (Exner, 1986), our preliminary conclusion was that anal rapists might have more suppressed aggression that might turn into hypersexualization and/or sexual perversion. And, 13% of anal rapists, but only 4% of vaginal rapists, could be diagnosed as sexual sadists, but these results did not turn out to be statisticallysignificant.
In our preliminary study, anal rapists were shown to have more sexual drive. They were found to be diagnosed by a hypersexual desire disorder significantly more often than vaginal rapists.
In summarizing our findings, we hypothesize that anal rapists are more sexualized. Compared to vaginal rapists, they are not more violent in general, but they tend to have more problems with unconscious anger, which consequently becomes sexualized during the offence, and, in turn, act out more aggressively during the offence: the anal penetration and more sadistic and violent behaviour during the offence.There is no doubt that these research data are preliminary and that more studies including bigger sample sizes must be conducted.

DISCUSSION

Compared to vaginal rapists, they are not more violent in general, but they tend to have more problems with unconscious anger, which consequently becomes sexualized during the offence, and, in turn, act out more aggressively during the offence: the anal penetration and more sadistic and violent behaviour during the offence.There is no doubt that these research data are preliminary and that more studies including bigger sample sizes must be conducted.

I liked the article since it didn't homogenize the rapists, as has been done repeatedly. Also, it posed straightforward empirical questions, what would seem to be indicated by the existing literature:
Is the anal rapist--
Sadistic? No** (not according to DSM IV 'sexual sadism' def)
Violent? Yes
Aggressive? No, not in self report, but supposedly yes by the cultish Rorshach measure of (hidden) "aggression potential"
Hypersexual? Yes

Reading a thread about female pubic hair got me thinking along these lines....
As a young bachelor; I find myself stunned and embittered as I contemplate the fact that, lately, I find the female organ of generation completely loathsome.

The words are too murky. There are at least two kinds of persons, the 'run of mill' dom (romdom) and the 'complete' dom, in your terms, rr, aka 'true dom.'
I'd rather drop the term 'dom' from serious discussion, since romdom is now the rule and approved type. (And I hate arguing what's the 'true', as in "Is Amstel lite a true beer?") This first kind is saying to the 'sub' candidate, "Would you like to play a chess match? If not, see ya. If so, I'll take White [role of issuing requests and orders already agreed to]".
The other kind of person can be called "alpha" as in the animal kingdom, or commander/commanding person or tyrant(petty or not). To those who don't resist (possibly because of training), the alpha says, "Do it." or "This is how it will happen." Perhaps that is the grain of truth in John J's position.
To those who resist, however, he says, "Do it or you will be punished or exiled (or killed)." As with humans, the exile involves the person losing something s/he considers vital (like love, community membership) and will be desperately miserable without. "Killing" for humans is not usually literal, but is a deprivation considered mortally serious; the emotional loss painful enough to lead a certain percent to suicide.
So I would say that for the alpha and the resister, "My way or the highway" is roughly correct.
There are endless nuances, and shadings; no blacks and white; there are gaps in the above. One topic not covered is training.
