You're right; homophobia does have to do with defined gender roles, about who's doing the fucking. If you "receive" then you're a girl (and the weaker sex), if you "insert" then you're the guy and the stronger sex. Among the Romans, for example, it was perfectly all right for a Roman Man to fuck another man, so long as that man wasn't a Roman, and was "receiving." It's that dominant position. Which is pretty much my point. It's all about pecking order and masculinity. A woman with a strap-on might or might not be a sexual turn on to a guy, but in the end, she's still a woman and the guy is (presumably) still bigger and stronger than her, and if it's going to happen, he's going to let it happen. But guys prove their power over other guys in fights, by overpowering them. They beat or wrestle each other to the ground. And if the winning male literally fucks the loser, that demonstrates dominance. It says, "I'm male and you're not!" It very much a visceral and animal level reaction.
Which isn't to say that a more effeminate gay man is going to be ignored by homophobics; I'm just discussing why, I think, sodomy is always discussed in regards to homosexuality and rarely, if ever, fellatio.
