The fact that people don't get caught doesn't mean it isn't happening. There probably isn't much cheating going on at the polls, but there could easily be with mail-in ballots. If somebody dies, that person will still receive a ballot in the mail. A surviving spouse can fill to that one and his or her own, thereby voting twice. Likewise new tenants in a rental. If the previous tenant receives ballots there, the current residents can fill them out and vote multiple times.

Probably fraudulent registrations, such as those from ACORN a few years ago, are a bigger problem. If the state's Secretary of State or the equivalent is dishonest, the bogus registrations will be accepted and some people are able to vote in multiple jurisdictions. Or, the registrations of people in opposing parties can be discarded, thereby disenfranchising members of the opposition.
