Exactly what constitutes free speach?

On the mall thing. You have no free speech rights in a mall. It is private property.

At the Mall of America about two years ago, there was a protest at Macy's because they sell fur. The protesters were hauled away, thrown in jail, and charged with trespassing. They were convicted. Their argument was a free speech one- they had the right to exercise their free speech rights in the mall. The judge said no. So did an appeals judge. Even though a significant amount of public money was spent on the mall, the protestors had no right to free speech within the mall

This is true at least for Minnesota.

Yelling fire in a crowded theatre does not actually harm someone. There is a PROBABILITY of harm, or even more specifically, the POSSIBILITY of harm. Yet, such an act is not protected by free speach rights. How likely must the harm be to lose the right to free speach?

You are right. The Supreme COurt has frequently ruled in favor of time and place restrictions on free speech. See my previous post about malls, though.
