HPI: patient states an ordinary day of waddling around when hit on the left side of the head by small pebble around 6pm. Collapsed and when regained consciousness, was unsure of self.

PMH:
1. Natural egg birth
2. Fully feathered at 9wks
3. Arthritis in right webbed foot.
4. Missing a few tail feathers

Medications / Allergies:
1. None

FHx:
Father: stuffed summer of 2011
Mother: sits with the chickens most days.
Siblings: (8) still waddling around with the peacocks and ginnies.

SHx: Unsure if married or offspring.

ROS: small bump on left side of head, scrape by lower bill.

PE:
Weight- 2.1kg
Height- 72 cm
Orange bill
Off white feathers
Round body
Webbed feet
Black eyes
Quacks

Assessment: feathered friend seems ok, a bit uptight with all the poking and questioning. Waddling around just fine now, able to swim around in small pool. Memory starting to come back.

Diagnosis: Duck.
