The condom is the way to go, every time, every partner until and unless you are truly prepared for both pregnancy AND an nasty STD or three.
I've gone without 4 times in my life and felt scared and guilty each time. It doesn't matter whether you cum in her or not; she can still get pregnant (from pre-cum, which is loaded with semen) and you are both at risk for STDs.
Hell, even in my sex dreams I use a condom! (Honest to God, in a DREAM I even stop to put one on!)
It's absolutely true that no combo of birth control is 100% effective. I have a friend who was conceived while her mom was on the pill, using an IUD (intra-uterine device), AND dad was wearing a condom! :O
