Sometimes threads are placed in the wrong spot.

I've seen a few misplaced threads that were acknowledged as "oops"es by the author and editor, especially where the thread was written off-line and then pasted into the wrong place. Almost happened to me once, and it's really hard to fix.

Have you seen this happen in other ways? (You add a new thread in one place, it appears in another?)
