6. The last person actually executed by firing squad was John Taylor in 1996 in Utah. There are still three inmates on death row in Utah who chose to die by firing squad prior to it's elimination as an option in Utah in 2004.

Oddly enough it was still a legal option into 2009 in Idaho and Oklahoma. Idaho passed a bill removing it in March 09 and Oklahoma only has it on record as a back up.... (if lethal injection is ever declared unconstitutional)
