How do I link a story in my story?

If I'm writing another of the infinite versions of “February Sucks”, or I'm writing a sequel to another author's story, and I want to insert a link to the original story in mine – then how do I do that?
The formatting for other hypertext stuff, like Italics and Bold, seems easy enough. But I haven't found info on inserting a link.

Do you have the author's permission to continue their story? If you don't have permission, you shouldn't. It's their story, not yours.
