I usually wake up with one. I get most of my erotic fantasies in dreams, and while I do get triggered by other things, usually I already have one going. (I also multitask, so I can keep working on a story while doing something else.) So, today was one of the usual days for me. I'm waiting for the one I submitted yesterday to get published before I finish it.

The first one was 2 girls having sex in a classroom, and pretty much went from there. The new one was a compulsive streaker finally talking to a co-worker she's never spoken to before. Sometimes they go where you'd expect, and others they even surprise me. It's just how I write, I'm weird, and as a conscequece my stories aren't all gushing with mass appeal.

Variety is more my thing, but now I know this thread is here, the next time I see something inspiring.
