A threesome can be anything you want it to be; it's your choice. There are no set-in-stone rules dictating one's sexual desires or relationships. i've been in both MFM & FFM combos. generally, i prefer the latter. the people were invariably my gf at the time and a friend. we didn't have ground rules per se, and honestly, that was probably a mistake on my part.
