The first thing you notice on someone?

Bellies...a really round one that I can punch or tap out a melody.
 
eyes, not only do I love womens eyes, but eyes in general indicate how a person reactes to you, they show fear, welcome, and if a person is a threat.

A persons voice... with an accent or without, doesn't matter... Someone who speaks eloquently is wonderful or someone with a think southern accent and actually uses the word 'reckon' in a sentence... There is just something about different voices that I notice before anything else...

depends on the woman...

if she passes into my line of sight, it could be any physical feature, so long as it is one that stands out. makes her stand aside from the crowd.
i could hear a voice and have to turn to see it.
i've even brushed up against a woman without looking and felt those "sparks and butterflies."
and every now and then, on the subway, my head is buried in a book. i'm not paying attention. i can't see, hear, or feel anything....
and then.....
that smell. mmmm...that smell.
