I have no idea what I clicked or if I did it but the tool bar above the reply box has all the icons grayed out except the one on the far right and the 3 dots. I would like to restore the icons if possible. Thanks.

I think I have the same problem but I can't find this toggle bbcode icon. Here's a screen shot of my comment box:

Your screen is too small, so it's hidden behind the three vertical dots on the right side. Click on those, then the "[ ]" icon to toggle BB codes shows up underneath.
