Some do. Some don't. Some use store bought enemas (fleet), some use enema kits that are a bit larger. Some do enemas as part of their ritual of strapon play, some do strap on as a part of their enema play.
Yes, most of the websites and how tos recommend cleaning your rectum with an enema. Some recommend larger enemas to cleanse more of your colon too. Typically, we don't have the time for enemas. We generally know when one of us is 'not empty'. And yep, sometimes we have a bit of a mess. It doesn't really affect us, though it makes for longer delays between fucking and cuddling. Gotta clean it up sooner than later, our opinion. Using condoms on your toys greatly speeds the cleanup process. Yes, there is also a health factor of interaction with fecal material - look it up, there are plenty of sites telling you why it's bad.
Be sane, be safe, take some precautions. If neither of you are terribly bothered by it, don't sweat it, but I do recommend immediate cleanup after you're done. Some toys stain easier than others and cleaning up quicker means less likelihood of any kind of microbes surviving and thriving on your toys before you play the next time.

It's definitely a great product. Be aware though that unless the female has an extremely strong grip and/or the male is pretty loose, you're going to run into problems with it sliding out of her without using a harness. The bulb (the end that goes into her) is pretty large but not so large that it can't just move right on out during intercourse.
The only reason we didn't use the harness was because she fingered me for awhile with heaven only knows how many fingers.
