Suction is your friend.

I've used syringes - got then in a pet store. Small sizes - the kind you'd use to feed a kitten meds or liquids. Cut off pointy end and sand down any rough edges. Use the plunger to suction up nipple.

I've heard rumors that snake bite kits can work for suction - easily available at Wal Mart or other sporting good stores. I believe there are different sizes.

I also recommend nipple suction cups and nipple twist suckers. I've been using suction on my nipples for years. Now, after suction, my nipples are huge and very sensitive. Be patient and don't leave them on for long until you get used to them. Initial overuse can cause painful water blisters. After extended regular suction, your nipples will stay extended. Then, if you want them bigger, get larger/longer cups or suckers. Both styles are available online or at your local ABS.
