He was on the brink of fame as the conductor of a symphony orchestra in Prague, but his career was derailed by his ether addiction and a scandal involving underage girls. Now he teaches music at a university. He's terribly bitter. Where does sex enter the story?

It's a guy making jokes on a television show, and that's it. We all say things for the sake of humor, and many of these things are at someone else's expense, negatively or positively (like we tend to jab each other here) alike.
But it's a joke, meant to make people laugh. Get over it.

Eddie Izzard's routine on Prince Philip is absolutely hysterical. He's taking the piss, but he's taking the piss out of things that Prince Philip has said ("Don't stay here too long, or you might end up all slitty-eyed") and done and he makes them hilarious.
From what I can see, David Letterman's whole joke is saying "Look everybody! Camilla isn't very pretty!"

I'm not judging anyone on the quality of thier jokes; we all drop one now and again that doesn't make people piss themselves, and now and again, at least someone thinks you've lost touch with funny. But c'mon, guys. The point was still to make people laugh, success or failure. Intent is what makes the difference.
I know a lot of people who'd listen to Eddie Izzard and wonder why the hell he's saying the things he does and what is so funny about a squirrel wearing make-up. But the guy's hilarious to other people (me included, though the only thing I've seen of his true comedy is "Dress to Kill"--feel free to toss out some more titles so I can look them up and check them out).
Look at it this way: Michael Jackson's on trial for something he may or may not have done. Has he been made fun of? I don't hear anyone coming to his defense, and he has done a lot of positive things for people. He's done more for charities than most people who ever even consider (though yes, others have done more--and they've still been made fun of). Why is it that the line should be drawn over this woman, who isn't really anyone except Charles new wife and long-time lover?
I'm not complaining to you venting here, just pointing out the truth of it. Comedy, like beauty, is in the eye of the beholder, for the most part.

Camilla's getting bombed for not being pretty, which a) she can do nothing about and b) isn't actually funny in any way shape or form.
The intent may be to make people laugh. But the result is to make people think that Letterman isn't funny.
Watch 'Have I got News For You'. Ian Hislop and Paul Merton will always have me killing myself laughing. They take anything and make it funny. Letterman is taking a well worn stereotype and playing it. No new spin, no amusement value, absolutely nothing to raise even a snigger.
There's no point in saying "Just don't laugh then," because if one person laughs, then he'll think he's worthwhile. I don't watch Letterman, I don't know Letterman, but if he peddles jokes of that quality, then he's not winning me over.
