As someone who lost my virginity to my mother, I'd echo what Adam says and say it varies. In our case, it was just the two of us. While it was and still is very taboo and illegal in some areas--even among consenting adults, it was private and discreet. Because we already knew and loved each other, the respect and trust were there.
