The best reference I've found on the subject is by Deborah Addington. It deals primarily with vaginal fisting from a woman's perspective. The author has experience doing it on both ends. The title is "A Hand in the Bush: The Fine Art of Vaginal Fisting". Amazon.com has it usually.
As for pre-stretching, it's ALWAYS a good idea to have the woman VERY turned on (I have used a combination of toys & oral sex and gradually built up the number of fingers) and use lots of lube. Another poster mentioned tapered dildos. They also make inflatable dildos. They're semi-rigid and relatively "standard" sized dildos that can be expqanded with a hand pump once inside for easier stretching.
As for wether it STAYS stretched, unless it's done routinely or improperly (causing tearing), the muscles will return to their original shape. The one time I have actually done this (I have participated in other scenes where it was done & so seen it several times as well), my partner was much more experienced. We had intercourse immediately afterwards and there was no feeling of "loosness". She said that, in her case, the tissues inside her vagina swelled immediately after, leaving her temporarily very tight and extremely sensetive.

Sometimes fisting is incredibly difficult. Plan on taking long amounts of time at a stretch when preparing and adjusting your body to it. I have found that it is better when you switch what you do to keep it interesting instead of an ongoing endeavor. For example, work on it a little bit, and then maybe do a little oral sex or anal play. Then go back to it and continue to switch what you do.
