Favorite BDSM checklist/questionnaire?

I'm looking for bdsm questionnaires for a story I'm writing, but I figured it'd make more sense to ask here instead of in the authors' hangout.

To be clear, this isn't because I need to learn about bdsm, or that I can't find one that I'm happy with. Rather, I'm trying to find some that aren't the ones that I, personally, would pick out.

Please help me out and point me in the direction of one that strikes your fancy, even if it's not one that you'd normally recommend to someone.

I think it's pretty interesting, and covers everything I could possibly think of, and more!!
