There may be a more medical answer, but for me it was the texture. A little thicker than your average drink, it really messed with me. My fix was discovered by accident - I started making homemade fruit smoothies for breakfast about two years ago and one of my favorites is made of banana, peanut butter, ice, and some other stuff (healthy, tasty, and portable). The consistency was similar to semen and the texture made me gag at first when swallowing but it tasted so good I made myself drink it anyway. The desire to do this should not be a measure of discomfort. If self play is resulting in that severe a gag reflex then I would personally recommend that you accept this as a limit for now, and work on oral stimulation versus deep throating. I would also ask your GP( Dr.) during a physical about your hyper gag reflex. It can be mediated with electrolyte treatment for dental purposes, and I have heard that some folks can use those sea sickness bands. These are repression only, and could wear off during play. If you ask your GP or dentist they might have additional ideas.

I would love to learn how to deep throat but I don't just have a gag reflex, I have a vomit reflex! I have the same problem when on my knees in front of B unless elevated somehow. Height difference does complicate things. Do you have stairs in your house? If He stands on the ground and you kneel a step or so up, it gives us just the right elevation to make up for the angle issue. We have a sunken living room and the single step up is the perfect boost for me if He stands in the living room. If we're on even elevation, it rarely works and different positioning is required (others already went over those possibilities).
