If a story is going for realism, pretending STDs don't exist ruins the suspension of disbelief for me as a reader. When I watch porn I know the actors have been recently STD tested, etc. On the other hand, if a story already is divulging a bit from the "real" world (fantasy, sci-fi, mind control, or a scenario so silly it would never actually happen) there is less need to acknowledge STDs and pregnancy risk.
If your characters are promiscuous and having a lot of sex with strangers it can be tough to have relatively safe condomless sex. On the other hand if your characters are only having sex with one or two people in the context of a story you can work a discussion in about protection before the action starts. Although obviously there are some STIs that can't be tested for, for me it is satisfying enough to have the characters exchange a quick little bit of dialog like:
"Should I bring protection?" I asked her.
"I'm on the pill and haven't gotten laid since my last test," she replied.
"Same here. Well, not the pill..." I stammered.
As a general rule of thumb, the sluttier a character is, the more they need to use protection, or at least imply they get STD tests as frequently as a porn star. I write a lot of group sex stories where unless you're dealing with an exclusive triad/foursome, safer sex is necessary. But in more romantic and monogamous tales I feel it can be avoided more easily.
