it could be just nerves or it maybe that when getting a blow job you dont think about the gender of the giver but when you try to top you cannot because you have a deep mental aversion and with your wife all is normal ,not sure if my theories are right but i hope this helps and i am sure others will have advice which may well be of a more sound nature .
