I also find this very hot, and I am not into Medical play either (although I have a huge crush on a woman who is, LOL). I think it has to do with the tension in the image, as you said. In all of the photographs of his that you posted, it is as though Newton has caught a moment, of both decision and indecision, where multiple choices are still possible.
