That's tough because it depends on what you mean by "nicest."
Ones that stick in my memory are often ones that exceeded my expectations. If you're paying $500+ a night for a room (as I did in Philly in January for a suite), you kind of expect it to be nice (it was...but honestly, I had to think to recall it).
On the other hand, sometimes the room itself is quite plain, but the hotel is interesting: I've stayed in some historic hotels that dated from the late 1800's or early 1900's when they were very opulent. The rooms themselves are often very small and not really very nice (hotel designs have changed dramatically, and there is only so much you can do in a remodel). But the lobby and the character of the hotel and the history make it wonderful.
I've stayed in some very nice, opulent, downtown hotels in cities like New York, San Francisco, and Washington DC. Marriots, Hiltons, etc. All are "very nice", very pricy, and I seldom remember them, or their name.
My favorite hotel experience, I think, was a little Pub in a little town in England.
The room was comfortable, and the pub was charming, as were the regulars who welcomed us, chatted with us, and recommended various ales. We had a lovely evening and a great breakfast the next morning. It wasn't fancy, it wasn't expensive but I will remember it all my life as one of the nicest places I've ever stayed.

About 3-4 times per year, on average. This year has been a little more.

I am...in about two weeks...headed to visit family in Florida.
Plus...myriad "vacations" next year...
Some would argue that all of next year will be a vacation...aaaaaannnnnd I 'm not sure they're wrong.

A few years ago the boys and I went rafting in the Grand Canyon. Unplugged, out with nature. A whole week reconnecting. Memories for a lifetime.

Went to Vegas years ago for a wedding. The two other girls I went with both got sick with food poisoning (along with like 10 other people...we all worked together and got sick from something at work the day we left). The bride had a death in her family the morning of the wedding and I broke my arm falling down a flight of stairs the morning after.
