Why yes I have...I was in college in the late 80s walking across the quad in my little denim mini skirt that zipped all the way up the front and my suede boots (and a sweater too!) and hit a wet spot on some brick. My leg went out (I didn't fall) and ripped the zipper completely out of my skirt. Ta Da! I quickly put my backpack in front of me holding my skirt up and went home!

I have plenty but one that involved a wardrobe malfunction and an embarrassing moment was when I was playing in a band. I was on stage in a small bar. I bent over and ripped some old jeans, and I wasn't wearing any underwear at the time so pretty much my ass was hanging out. Mercifully I had a change of clothes in the van, so I had to step away mid-song and change. I got quite the applause from the 5 or so friends in the bar when I returned.
