The Galloping Ghost, Reno air races 2011. The rudder trim tab departed the plane causing a sudden and fatal pitch up event, the plane rolled, and went into the ground on the ramp. Several people were killed including the pilot, many were injured. The NTSB report said the reason the rudder trim tab departed was due to a reused nylock nut. The nut came off, and so did the trim tab. On one side, it could be argued that it was poor maintenance and pre race testing, on the other, it has happened once before. The difference was, the first time it happened the plane shot straight up, pilot blacked out, the pilot came too well above 10k feet, pulled the power back, called a mayday and landed the plane.

This incident was the first time a spectator had been hurt, let alone killed at the races in 50 years.
