One thing that annoys me to no end is when people attempt to speak to me when I have headphones in. At first, I can't hear them; then when they speak louder to attract my attention, they get pissed off. Of course, this is only followed by me pausing my music, and by this time, they've repeated themselves thrice and don't want to say it again.

A simple tap on the shoulder in the first place eliminates all of this.
