From talking with female Dominants/Mistresses back in the days when I was active with a pretty pansexual group, it depends (like so many things) on the individual(s) involved. Some got what they described as "mental orgasm" from the act of using a strap-on on others; some got physical pleasure from the movement, pressure, etc., of the strap-on and its harness pressing against the right spot(s); and others, using a strap-on with an insertable (to them) portion, felt they had the best of all possible worlds, both fucking and being fucked.

I prefer a harness anyway, because to keep the thing aimed forward -- both inside and out-- I have to keep my thighs locked tight. And I guess the feeling of the harness has become integral to my strapon pleasure or something...

I just learned to tie a rope harness and I love that-- I can adjust where the pressure goes.
