I have seen standard of care chemotherapy be denied, requiring appeal that delayed cancer care by weeks. It's unconscionable.

I had an EFT with 11 claims and 8 were denied for the code 99214. Seriously wtf?! And I hate the policies that don’t cover preventative care.

99214 is so absurdly easy to get to too. 2 out of 3 of 2+ chronic stable problems, prescription medication (including continue current meds at current doses), or review 3 pieces of data. Like over 95% of primary care visits should be a 99214 or 99215.
