It can be 1. Since she's a professional chiropractor, I'd say keep the fantasies to yourself and never let on you think anything different. I see a chiropractor and get twisted in various positions every time I go, but this is their profession. If you mentioned it, there would be several awkward seconds afterwards, you'd excuse yourself and she wouldn't think anything more about it.
Now, I'd be the first to potentially read into something more, given the same situation, but that's where you separate fantasy from reality. In all likelihood, she does this all day to hundreds of patients and never thinks twice.

I have a hard time thinking about sex at the chiropractor, but then never a nice looking female Chiro.

The first time I went it was funny, I am a big guy 6'2" 240 and by back was completely in spasm. The guy who was almost my size got me all twisted up and then crunched me, he was actually grunting when my back finally cracked.

I was surprised by the sound, and said "fuck me", when I apologized he laughed and said it was probably an appropriate thing for me to say given how hard he had to work to crack my back.

We both laughed, but not something that would have been sexy with the most beautiful woman in the world.
