Getting an infection is actually pretty rare.

There is a medical condition which prevents the foreskin from fully retracting which requires circumcision. However, I doubt your lover has that problem . Well he does pull it back when he goes to the restroom or to wash it. He's very clean there, so that's not an issue. It was just when I was pulling on it...guess I did it too hard and too far.

Even when he's hard, the extra skin is still able to slip over the tip of his cock. It doesn't cover the tip completely once it's hard, but it does still slip over the ridge a bit. It wouldn't have to do with the size of your penis. Generally the girth of your head, the lenght and elasticity of the frenulum (what connects the foreskin to the head of the penis), and the amount of foreskin would determine if your foreskin just automatically retracts or not.

Personally when I'm erect the skin still covers the head (I would take a picture but I'm camera shy ). Once I start having intercourse the walls of the vagina make it retract and it stays that way until my penis becomes flacid again (or I manually move it back).

The skin going back and forth over the head fells pretty good once you're into it, it's like a built in masturbation sleeve really. Of course that shouldn't happen when having intercourse.
