Many of the things people have mentioned previously, like smoking, rudeness, abusing the help, being cheap liking, annoying music, bad hygiene, and being inconsiderate. I'll add a few of my own.

Being able to see someone's ribs or hip bones.

Overly picky eaters.

Yelling on your cell phone or having a REALLY LOUD ring tone.

Thick makeup.

Heavy perfume or cologne.

For me, it's not tighty-whiteys per se, it's when they're all stretched out with blown elastic, and they clearly belong in the trash. A fresh, neat, newish pair of the whiteys isn't a turn off, but I can't say I'm particularly attracted to them, either.
