Interesting topic. For me, it's a mix. The location, jobs, cars and so on that I describe are completely true and are taken from my real life. Characters are based on people I've known, know now, or have read about. The sex is almost entirely made up, especially the BDSM bits, which are entirely a figment of my imagination.
As some else commented, it's more important that characters act in a way that's vaguely consistent with how they're described. I don't enjoy, and try not to write, stories that aren't character driven.
