This is one of my favorite fantasies (both with wife and my mom, and wife and her mom). Does it happen in real life? Probably almost never. It is believable as a story? Here, anything is believable if we do the work to make it so.

There is a deep beauty in both themes.

With wife and her mother - it is the idea that these two women have decided to both love the same man and share him with one another, not threatened by each other but having the experience enhanced by their relationship. There is a sense of conquest from the man's perspective in taking two pussies, one of which is the product of the other, for himself - conquering a "female line" with his masculinity. Well, the obvious challenge is to make the threesome believable in terms of relationships between the partners. I would personally think the bar is much lower for wife and her mother than for wife and his mother, but both could be constructed with some creativity.

Wife and her mother could be fMF focused on the guy, while wife and his mother perhaps would rather be MfF focused on the younger woman, but a lot depends on the genesis of the threesome.

It could be established mother+son couple searching for an acceptable girl to add for long term concealment of their relationship and/or for the son to procreate with. It could be hubby's mother developing irresistible lust for his bride. Or it could be the wife going on a campaign to seduce MIL, perhaps in misconstrued belief a concealed sexual relationship between husband and his mother existed, or in thinking a good fuck is what's needed to repair relationship between hubby and his mother. Each would have very different internal dynamics.

It could be the classic "came for the daughter, stayed for the mother" situation where relationship with girlfriend's mother grow deeper and more meaningful than the perhaps physical sex focused relationship with the daughter, either very early on or eventually, maybe many years later. In mother/son scenario, they could have incestous relationship before, and wife kind of joins as a third person. Son/husband may be momma boy and his wife wants him to grow out of it, or he wants out and mom tries to keep him for any cost.

Tbh I think the same as for any story I mean if someone is repulsed by such idea, you can't win them over. And if someone us into it, they just want a well written story.

The widowed mother-in-law moving into the home is definitely the most realistic setup. Maybe the husband works from home and the wife works outside of the home. This happens a lot now. I have a little plot I came up with sitting here:

MIL and husband start off as a very innocent relationship. But as the months go by she is making lunch for him every day, he is changing her oil, helping throw her clothes in the laundry. They relax and eat lunch together every day, slowly getting more and more comfortable with one another until they realize that although husband and wife have a great relationship, husband and MIL have a deep friendship that is growing into love. It's mature and real and full of shared interests. Husband feels like he's just talking to an older, more experienced version of his wife but the same smile, eyes, beauty, grace, elegance and sexuality exude from this woman. MIL feels younger, more adventurous, and happy in getting to know her son-in-law. She's proud of her daughter and happy for her, but also knows that deep inside there's a desire - both emotional and physical - that has awakened as she has become closer to him.

They're sitting on the couch one day watching TV after lunch, husband has a meeting in an hour on Zoom, and they find themselves cuddling. It could progress from here into a guilty kiss followed by an apology. And then later into an embrace followed by yet another apology. Eventually turning into an affair. It is if you write it in a believable style. Many use the excuse of the improbability of it occurring as an excuse to write it clearly as over the top fiction.

Does it happen in real life?

Research is sparse as there's little financial incentive (and it's a subject that wouldn't be openly discussed) but sheer probabilities suggest it is rare but not unheard of. It is not unreasonable to write about it from a possibility lens/mindset. And would likely have more realistic seemingly narrative outcomes.(if that's your goal)

But it's rare enough I would consider it your one big reader ask to "suspend disbelief." I would be very careful to make most other choices very milquetoast so as not to push readers over the wall. Mom working for NASA or wife a porn star, everyone built like Adonis/time defying goddesses, similar extra improbabilities invite the reader to use even more of a critical eye.

What are the challenges in writing a good story with this theme?

Same as most incest with the exponential challenges of incorporating polyamorous relationships within the complexity of a socially taboo/shunned relationship.

If it's more simplistic "wife lets Mom and son still play" you have an open relationship+ so address the taboo difficulties as you would in standard incest then cover wife's feelings about who this makes her husband and, of course, the standard open relationships challenges.

If it's a true poly, which most people go for/expect when the significance of family is added in, you've got to address all legs of the triangle.

It's a high degree of difficulty but so too would be any true exploration of poly which you are adding another serious twist on top of.

Agreed but there are also levels in my estimation. Simple boy meets girl but brother sister isn't a big ask, especially in category.

Polyamory, threesomes, wives and mothers up the ante considerably.

Every reader has their own line but I find a big plausibility ask then retaining mostly modesty in further asks is better practices if overall plausibility is the goal. Big ask shines a spotlight hard to navigate through with similar improbable scenarios.
