It never occurred to me to have any fears, as I'd only participate if I were totally comfortable with both partners and our various dynamics.
I don't think I'd be much for just watching, but I would be okay taking a supportive role for a bit as long as there were something for me to do - kissing, stroking, licking, etc. Is it really very likely that a third partner could get left out entirely with so many parts and appendages in play?

My first experience with a threesome I would classify as an "unexpected threesome." We had talked about it, that it was something she wanted to happen, but not the when and the how. So I was a bit surprised when, as she was finishing me off, her husband was suddenly there. I felt really awkward and exposed, and excused myself to clean up. Only later did I learn that he had been in the room the whole time, watching. I know it wasn't exactly as anyone had planned, but I know she enjoyed it. So I'm hoping that in the future, we can coordinate a little better, and plan ahead. I'd also like to see what would happen adding another woman into the dynamic.

Performance issues:
In an MFM, performance issues would be bad but not devastating. In an FMF, after closing the deal on making this happen, performance issues would be horrific.
