This must be from when she was skinny, because she is sloppy fat now. She ostensibly can't find any clothes that fit, so she walks around in her underwear which is unfortunate for me. If a medical doctor is using slang at all, I’d suggest going to a different medical doctor. You want to get real technical they’d use the term obese, not fat.
