Everyone hates spam callers. Some people love messing with them, and can drag out the call.

What if, you start by dragging things out, but it starts a journey to flirtation, and then to something more intimate?
 
Problem is the spam caller will likely be in Bangalore or Manila so physical intimacy might be a challenge. In the olden pre-digital days I hear that crossed telephone lines sometimes resulted in unexpected friendships and more. Anyone experience that?

Three possible approaches, off the top of my head:

a) Just have it be a phone sex tale. There's nothing that says your characters have to actual meet, for you to be able to get off the reader.

b) Go left-field. What if your spam caller is a literal demon? It wouldn't be out of character for hell to be torturing the living by asking about car insurance.

c) Start the tale with phone sex, and evolve it into a longer romance where a long distance relationship eventually involves someone getting on a plane.
 
Maybe it can one of those sextortion scams where the caller seems to know everything about the person they are calling, all of their bad habits. If they're a demon they can know everything, but clever scammers use generic information and clever questions to convince you that they have already hacked your computer and your phone.
 
Maybe it starts off as a phone sex type of call, and the caller gets the callee all horned up with lots of nasty, dirty talk. And just when the person is about to go out of their mind needing to orgasm, then the caller reveals themselves as really selling extended warranties for cars and gets them to agree when they are most vulnerable.

Ah, microtransactions. Phone sex lines are sort of the origin of that kind of financial exploitation, so it fits well.

It could also be a great humorous punchline, if setup and timed well.
 
How about this:

A spam call leads to phone sex. The called one (the man probably) wants to meet the (female) caller. She promises it to him - but only if he himself does at least five spam calls. So this is a kind of network marketing to get others in line...
