The Spiky Thing

The Spiky thing in Cym's Avatar is called a Wartenberg Pinwheel. And, like cym said it is used in the vanilla world by folks in the medical world.

I love mine, and I use it a lot on my boy. it is great to use in conjunction with other tools for sensation play. It is especially effective if the sub is in sensory deprivation.

For example:

I usually tie my sub spreadeagled on the bed. He is gagged, blindfolded and wears earphones playing soft music.

I then use a succession of objects on parts of his body, feathers, ice, the pinwheel, paddles flogger, clothespins, etc to name a few. Sometimes I even use some CBT ( no longer than 10 minutes) with just the head of his cock exposed. I use the pinwheel very lightly on his inner thighs, scrotum, feet, everywhere. I alternate intensities, even using tickling.

To make a long story short, after all of this, he is begging for release.

I know its a medical device, but you can also buy them in any fabric/craft store. They are used to transfer patterns or pattern markings from a pattern, using carbon paper, directly onto the fabric. I can't remember the name of it now, but it might be more accessible at your local JoAnn's and a little less expensive than hunting down a medical supply store.

Yep, I forgot what they are called too, but the medical one is a lot sharper, I think.
