Get a booklet where you can write down all this info. Amazon has them. Most of us have so many accounts with passwords and user names that it's impossible to keep this stuff in your head. With gmail, Google will save this stuff for you too. Nevertheless, have it on paper too! Old fashioned but it works.

Your situation is a little different. You have access to that email but don't have the login/username for the account. I think that if you used that form, though, then they could probably help you "regain access to your account," which is what that FAQ page is about.
