I succeed far from every time, but my general approach is to answer the question "what information is missing between the title and the category to give readers an idea of what the story is about but without giving away the end?"
Here is an example I think worked pretty well:
Title: Unusual Dinner Party.
Category: Exhibitionist & Voyeur.
The title already gives away a lot. The story takes place at a dinner party, suggesting it's going to involve a group of people who know each other. The "unusual" suggests this particular dinner party ends up being out of the ordinary, so it's about something that these group of friends don't normally do. The category suggests the unusual aspect is related to some sort of exposure/watching. Important information missing is that it's a story with a lot of spanking, and I also wanted to convey that it had a sex-positive vibe. Thus...
Tag line: A spanking good time with dinner guests.
Here's an example that's missing something:
Title: What She Deserves
Category: Loving Wives
Tag line: Away from her husband, Fiona meets a stranger.
Between the category and the tag line, it should be pretty obvious that it's about a married woman having sex with someone other than her husband, and the husband isn't present. That's all good, but the title is suggesting it might be some sort of BTB scenario, which it is not. A better tag line (or title) could have remedied that.
