I personally object to any refusal of insurance to pay for treatment of a patient who has consult with their doctor.

Insurance should pay for our health and should not be subject to scrutiny. That being said, certainly treatment which is being made against advice of doctors or procedures that are outside of health decisions should require an additional premium from the patient.
