Very sorry if these are answered elsewhere, but I legit couldn't find the answers:

1. What are the exact criteria for getting the red H?

2. Is a story's rating a simple average or is it weighted in any way?

The site tries to counteract the obvious deficiencies of this system by conducting "vote sweeps," where votes deemed fraudulent are removed. I'm told it used to be done more or less regularly, but right now it basically only happens around the official contests and doesn't affect anyone other than particupants in said contests.

So yeah, it's not great.

One correction. A sweep impacts all stories, not just contest entries. I have definitely had non contest stories affected by a sweep. I believe it looks at all votes over some time frame.

Were you participating in the contest when that happened?

I'm asking because I never had my stories affected by a sweep unless I had an entry in it. Sure, my non-contest stories would sometimes get hit, too, but I haven't heard about a random non-participating author to be affected by a contest sweep.
 
I have only participated in two contests, but I have noticed changes in ratings for all five sets of sweeps that have happened since I started writing here. Sp yes, I have seen changes when I was not a contest participant. Some before I had ever been a contest entrant.

Anyone can request a sweep on one of their stories by using the report feature on it and asking in the notes field. It will get done but these days I don't know how long it can take.
 
Any story can be affected by any sweep. ( <-Absolutely Positive | Reasonably Certain -> ) Stories other than the target group which have votes swept is a fringe benefit of bad actors discovered during the sweep. It's a secondary sweep of the whole database purging the scumbags discovered.

Don't think that vague information can really help trolls. LOL
