Partner/spouse undergoes a sudden, severe physical transformation and their partner/spouse isn’t sure of how to deal with it.

For example:

1. Older husband who was on his deathbed suddenly comes to the next morning as a young Adonis. Tests confirm it is him but doctors can’t explain it. Wife had gotten used to hubby being close to death. Was she a Gold digger, or had she just gotten used to a sexsless marriage. Or is she also old and doesn’t know how to keep up with her physically younger husband

2. Astronauts on the first mission to mars hook up and upon landing, one of them is caught in an explosion from a boring probe. The others are sure they died but suddenly from their suit emerges a strange creature that begins terraforming the planet and trying to get them to come outside.

Stranded on another planet the supposed to be dead astronaut’s partner is instructed to keep up the relationship for the purposes of making mars safe. Though it becomes clear the creature wants an intimate relationship like they had before

3. MC is living in the post apocalypse with her girlfriend. Lately MC has become worried about her: she seems to jump in fright every time MC comes into a room unannounced and she seems smaller. But tonight MC is going to make a big meal, put on some lingerie, and fix things up with her

(In actuality, MC has become a hulking mass of tentacles woven into a humanoid form but is oblivious to this change)

My Halloween contest one will have the husband and wife stay as such, but are transformed by a demoness that is “doubly equipped”.

Astronaut ships out on an extended mission. Unknowingly, his craft drifts through an asteroid belt and he picks up a symbiote. It's fully incorporated with him by the time he lands on Earth. His wife, the little tart, has been having several affairs in his absence. He arrives home to find her in the middle of one and, in a rage, sprouts tentacles. He takes his wife, as tentacle monsters will, and she loves it so much she dumps all her other boyfriends.

But the silly woman gushes to all her girlfriends when they go out drinking one night. So now, they all scheme to get a piece of that tentacle action. He figures he's due, and when they avail themselves of his slimy charms, he takes each one in an alien fashion. ie...rough and hard and for a long time.

Soon, he has a nifty little harem, which includes her sisters and Milfy mother.
