Over 1000 posts, no user title box

I'm over 1000 posts, I've logged out, cleared my cookies, and logged back in, closed out the browser, left the site for ten minutes and all, and I STILL don't have my custom title box. Why is this?

Does it do it automaticly, or does the admin staff have to enable it for each user first?
