It's a temperal rift, her existance is causing a clash of his life, blending it with touches of his life in an alternate reality. Everytime she's nearby the parrallel worlds collide on his head and her physical body combined as a merge point. Because of Mercury retrograde. She's a displaced temperal anomoly existing where she shouldn't. They spend an evening together, or what seems like an evening ending in sex, as they get closer to climax, his mind and her body start to fade from existance smashing the two realities together. He wakes up alone with faded memories that aren't his of a woman he thought he was with. Every time he sees her the outcome is a sensation of deep passion as their pairing is the destruction of two universes and they only percieve it as raw passion.

Okay, then, she was his nurse. After a horrible car accident, he spent three months in a coma*. She was the night nurse in his ward, and she spent every night wiping his ass and massaging his limbs and taking his temperature. She became smitten with his muscular body and handsome face. Eventually he was discharged, but it was during the day. He never saw her while he was awake, but he subconsciously observed enough of her for her to seem very, very familiar. And very very important to him.

*If you want to make it more topical, put him in a medically-induced coma and on a respirator suffering with Covid. I have a friend who spent two weeks like that.
