It's been a while since I read it, but if I remember correctly it's from the sister's POV. Early in the story it mentioned that she was institutionalized for being able to see ghosts/monsters/some type of entity. Sister comes back home from college I think (I can't remember if it's from college or somewhere else) and starts seeing the non-human antagonist that's some sort of ghost/monster/demon creature
Brother finds sister covered in bruises later on and barges into her room and later gains the ability to see the antagonist. Antagonist reveals that brother has had feelings for the sister and then the two are forced together
